Reminders for Your Visit:
- Please complete our patient forms prior to your appointment. If you are unable to complete our forms prior to your visit, please arrive 10-15 minutes early so you can complete them in our office.
- Bring your dental insurance card and ID, along with any dental records or referrals
- Bring a list of any current medications you are taking
- Patients under the age of 18 should bring their parent or legal guardian to the appointment
- If you need antibiotics before dental work, please take them as prescribed. If you are unsure whether you need antibiotics, please consult your doctor before your visit.
